**DO NOT include LIVING PERSONS on these charts.**

The form below is made for a family of 10 children.  If you have more than 10 children to a set of parents, please submit a second form with both the father's and mother's names (or just the mother's name if she was unmarried), with a notation in the comments box that states this is additional children to the previous 10.  To simplify organizing these and prevent duplication, the chart will only be listed on the Family Group Sheets Index page under the head-of-household's name.  To make your submission more complete, please also submit a separate chart on the mother's family.  If there is more than one marriage, submit a second family group sheet and make a notation in the comments box that states this is an additional marriage.

Use your Tab key or mouse to navigate between fields, the Enter key will submit the record in newer browsers.  If this happens, use your browser's "Back" button to return to your record.

1. DO NOT include any LIVING PERSON on the chart.
2. Please type all SURNAMES in capital letters but don't type the first name(s) in all capitals.
3. Dates should be in the format of MM/DD/YYYY if you have full dates, MM/YYYY if you only know month and year.  And YYYY if you only know the year.  If you only have an approximate date, please submit it using the notation "ca."  Always use a 4-digit year (ie. 1900 not 00 or '00).  You may also use the format dd month yyyy (ie. 1 Jan 1900)
4. You may add any additional information in the comments box.  If you have them, please include items such as military service, occupation, church affiliation, race, etc.  If it is not obvious how the family was related to Kaufman County, please make a notation about that here. Remember: each family must show a connection to Kaufman County, Texas.
5. Please quote your sources in the sources box as where the information was found is extremely important!  Without the source information, there is no way to verify it nor to check the sources when conflicting information is found.  You may wish to number or otherwise label the sources to make it more clear which source goes with which piece of information.
6. Be sure to enter your name, e-mail address and sources as these are REQUIRED.

If you are making corrections or additions to existing families you have already submitted, you may either submit a new family group sheet or enter the father's name (or mother's name if she had no husband) and the corrected and/or updated information only and leave all other information blank.  When submitting only updated or corrected info, please state this in the comments box.

If you would like to prepare your group sheet off-line, contact The Coordinator for instructions.

Please DO NOT push the "Submit" button more than once for each group sheet.
